PRINTABLE REFERRAL FORMS:
You will need to ask your doctor to fill out a referral to one of our sleep investigation units for a sleep study. You can download one of our referral forms from the link below on this page. Once your doctor has filled out a referral, you should phone our head office on freecall 1800 155 225 to book into a sleep investigation unit in your area.
The results from your sleep study will take approximately 10-14 working days to be analysed and reported by a Sleep Physician. After this time you should make an appointment with your referring doctor or Sleep Physician to discuss the results of your sleep study. You may be required to return to the sleep investigation unit for further testing or you may be referred on to other Physicians.
